Description
Link for Care is a community service for caregivers, individuals needing personal assistance, healthcare professionals, social workers, discharge planners, and mental health professionals in Kansas City. Link for Care can be accessed at www.LinkforCare.org. Link for Care is maintained and managed by the Central Plains Geriatric Staff at the University of Kansas Medical Center, Landon Center on Aging, and was developed in part with funding from the Department of Veteran's Affairs and the Department of Transportation. MARC is also a partner.
